The short version: this site has no server that receives anything you type, and there is no account to create.

What happens to your answers

The questionnaire runs entirely in your browser. Your answers are held in this tab's session storage so that moving between blocks does not lose them, and they are discarded when you close the tab. They are never transmitted anywhere, because there is nowhere for them to go: the site is static files and contains no code that sends a request carrying your answers.

Your plan

Your plan is assembled on your own machine from your own answers and rendered in the page. Saving or printing it uses your browser's own print function. There is no email step on this site, so no health information passes through any third party.

What is measured

The site uses a privacy-preserving analytics beacon that counts page views. It sets no cookie, does not fingerprint your device, and cannot see anything you type. Nothing it records identifies you.

Fonts

Two typefaces load from Google Fonts, which means Google receives the request for those font files, including your IP address, in the same way any third-party asset does.
